The Assessment Appeals Process
You should complete and submit this online form if:
- You are prevented from attending any form of assessment task (including examinations) due to illness or unforeseen misadventure.
- You consider that your performance in an assessment task has been affected by illness or misadventure during the assessment task.
If you were absent from the task this form must be completed and submitted immediately upon your return to school. If you consider that your performance in an assessment task has been affected by illness or misadventure immediately before or during the assessment task, you must make your teacher aware of your intention to lodge an appeal prior to the commencement of the task.

Guidelines for Illness/Misadventure Documentation due to COVID-19
COVID-19 continues to disrupt student’s ability to participate in their scheduled assessment program. Whether a student tests positive for COVID-19 or is forced to isolate due to living in a household with someone who tests positive, such occurrences need to be treated in the same way as any other incident that impacts a student’s ability to participate in an assessment.
Any Year 10, Year 11 and Year 12 student whose participation in a scheduled assessment task is impacted by COVID-19, whether it be personally testing positive with COVID-19, or being the household member of someone who has tested positive, must submit an application for illness/misadventure and include a copy of one of the following:
- A screenshot of the registration of a RAHT with NSW Health confirming the positive test result
- A copy of PCR test report
A separate illness/misadventure application needs to be submitted for every missed scheduled assessment task.
